Continuing Education Sponsor - One Time
The application form must be completed and returned to the Board office. If possible, please send this application 90 days prior to the date of the event. The Board will review your application and you will receive the results shortly thereafter.
If your organization wishes to become a permanent sponsor of continuing education and receive Board approval for all pertinent CE offerings during the calendar year, please complete a permanent CE Sponsor Application form after you have completed three one-time, Board-approved CE offerings .
